Individual Support Services
It is increasingly difficult for disabled and disadvantaged people, who do not have a care package to access support which would allow them to take up opportunities or go out independently. The Individual Support Service aims to address this issue by providing support to adults who have a physical or sensory disability and are members of Adapt (North East) who can book a support worker and accessible transport.
The Individual Support Service provides ad-hoc support for people to access services in the community which could include visiting the hairdresser, library, leisure centre or for shopping trips, however this service does not provide personal care.
This service is available Monday to Friday 9 a.m. to 5 p.m. and should be booked in advance. The cost of this service is £3 per session with the length of a session being 90 mintues and accessible transport costs 30 pence per mile.
Please note this service does not cover food shopping, we would however be interested to hear from you if you need transport to help access supermarkets in the area.
